Course Participant Agreement.
Public reference only. The signature copy is sent through DocuSign or PandaDoc and identifies the participant, course, campus and confirmed role.

Parties, course and role
The signature copy identifies OralAdvance Labs d.o.o., the participant, dates, host clinic, clinical director and exact role. Clinic details may be added later while a venue is being secured, but no operator role exists until all required details and approvals are complete and the role is confirmed in writing.
Price and payment
- Group: €8,900 total; €2,700 reservation and €6,200 balance.
- Private: €16,900 total; €4,700 reservation and €12,200 balance.
The balance and required documents are normally due four weeks before the course. Payment is separate from the operator-role decision. Stripe collects payment only; the legal VAT invoice is issued separately.
Documents, insurance and continuing validity
The participant warrants that identity, qualification, registration, good-standing and insurance evidence is authentic, accurate, complete and current. Good-standing evidence must meet the stated 90-day validity rule, and registration and insurance must remain valid through the course dates. Changes must be reported immediately.
Safety authority and professional responsibility
The clinic and clinical director may approve case allocation, pause, modify, stop or take over treatment, or remove a participant from clinical activity for safety or compliance. Attendance does not create a licence. Each person retains the responsibilities assigned by applicable law; this agreement does not automatically transfer all liability to the clinical director.
Records, follow-up and adverse events
The campus-specific signature copy identifies responsibility for patient sourcing and screening, records, sterilization, emergency response, postoperative follow-up, complication management and adverse-event reporting. The participant must cooperate promptly.
Refund, transfer and cancellation
The signed agreement incorporates the Refund Policy, including genuine clearance failure, missed document deadlines, fraudulent documents, one transfer within 12 months and mandatory consumer rights.
Privacy and separate media consent
Professional documents are stored securely and disclosed only as described in the Privacy Policy. Participant media consent and patient marketing/publication consent are separate and optional. Treatment cannot depend on marketing consent.
